For your pet’s protection, all vaccines must be current or vaccines will be administered at owners expense. Vaccines required for boarding include Rabies, Distemper Parvo Leptospirosis, Bordetella, and Canine Flu. Your pet must be free of internal and external parasites. If not, treatment will be performed at your expense. The kennel is not responsible for the loss of any personal belongings left with your pet, though we take precautions to keep everything with the patient(s). Accommodations: Accommodations in our specially designed lodging suited to the size of your pet are, unless otherwise requested. Your pets quarters will be cleaned and sanitized at least twice daily. Twice daily feeding with Royal Canin Sensitive Stomach; fresh water will be available at all times; feline friends are provided with a bedded kennel and a place to hide; canine friends will be exercised 2-3 times each day on leash in our pet relief area; treats are occasionally given to pets, unless specified not to give them anything other than their normal diet; daily medications brought from home will be administered according to directions and may incur an additional fee.

*When your pet returns home, please do not allow him/her to eat or drink excessively. This is a common mistake and often causes vomiting and/or diarrhea. Wait at least an hour before giving a small portion of food or water. Please call 817-523-7210 if you have any questions.* If my pet(s) become overly anxious during their stay with us and a tranquilizer and/or a sedative is necessary, I understand the medication(s) will need to be administered to improve my pets stay. I hereby give permission to the staff doctor(s) to administer medication at my expense. Pets must be picked up during normal business hours, (Monday-Friday, 7:30am-5:30pm). Walnut Creek Veterinary Hospital will not discharge any pets after business hours, on Sundays, holidays or when the clinic is closed. The clinic cannot guarantee the health of any animal, but pledges to give appropriate care to all pets who board at the facility. I agree to hold Walnut Creek Veterinary Hospital harmless for any medical issues that are often unavoidable in boarding environments including but not limited to: weight loss, loss of appetite, rough/thin hair coat, kennel cough, upper respiratory infections, vomiting and diarrhea. If the condition is life threatening or contagious, supportive treatment will be performed and the charges will be added to my invoice. I agree to make full payment to the hospital at the time of discharge. Intact males and females, while strongly encouraged to be altered, should they be in our boarding facility and an unforeseeable interaction take place and your pet becomes pregnant, Walnut Creek Veterinary Hospital will not be held responsible. We will take every precaution necessary to prevent such acts from taking place, however, there are possibilities of such acts occurring. If, upon my request, my pets are put in the same kennel or run and an unforeseeable action occurs i.e. food bowl aggression, fights resulting in lacerations and/or death, I agree to hold Walnut Creek Veterinary Hospital harmless and all supportive treatments will be added to my invoice and full payment will be expected.
While we strongly discourage and highly recommend not boarding puppies and kittens until they have had their full series of vaccines, they can be boarded at the clients discretion. Agreeing to hold Walnut Creek Veterinary Hospital harmless should my pet become ill from a contagious or deadly disease. The hospital routinely has sick, contagious and deadly diseases in their facility and your pet could potentially be exposed. Regardless of the staff taking every precautionary measure possible to prevent spreading of contagious diseases; your pet could potentially be exposed. If my pet becomes ill during their stay, Walnut Creek Veterinary Hospital will treat your pet accordingly and full payments to the hospital will be expected at time of discharge. I understand if I fail to pick up my pet, I will be contacted by telephone. If I have not made contact and payment arrangements to the hospital within 5 days of agreed discharge date, a certified letter will be mailed to my address. I understand I will be given 10 days, starting the day of mailed notice, to retrieve my pet and satisfy my financial obligations with the hospital. If I do not make payments to retrieve my pet or to extend the stay, I understand that my pet will be considered as "abandoned" in accordance with subsection §801.357 of the Veterinary Practice Act, and will be transferred to the local Animal Control Facility. *Finance charges include: Minimum 1.5% or $5, whichever is greater on balances over 30 days. I agree to make full payment at the time of discharge. I additionally certify that my pet appears to be free of contagious disease(s) and has not bitten anyone in the last 10 days I have read and fully understand the terms and conditions set forth above.
